Day to day

What fellows do

  • Scan patients at the bedside in the emergency department.
  • Log every scan in the QA platform, with their own interpretation.
  • Perform ultrasound-guided procedures and log them.
  • Read and act on the written feedback faculty send on their scans.
  • Act as seniors for KBEM residents on rotation, confirming the residents' scans.

Quality assurance

How review and feedback work

Every scan follows the same path, from the bedside to the outcome. Faculty read each clip before they see the fellow's answer.

  1. Scan

    The fellow scans the patient in the emergency department and saves the clip.

  2. Log

    The fellow logs the scan in the QA platform with their own interpretation.

  3. Blind read

    A faculty member reviews the clip and records their own read, without seeing the fellow's.

  4. Reveal and score

    The fellow's interpretation is revealed. Faculty score entrustment from 1 to 5.

  5. Feedback

    Faculty send written teaching feedback to the fellow.

  6. Outcome

    Accuracy is tracked against what happened next: CT, surgery or the clinical course.

Why blind? Reading the clip before seeing the fellow's answer keeps the faculty read independent of it.

Scan target

1,500 scans over the fellowship

The program target is 1,500 scans over the fellowship, each logged with the fellow's own interpretation.

Assessment

How fellows are assessed

Entrustment

Faculty score each reviewed scan for entrustment on a scale of 1 to 5.

Accuracy against outcomes

Fellows' interpretations are checked against outcomes: CT, surgery and the clinical course. This builds a record of how accurate each fellow's reads are.

Competence committee

The competence committee reviews each fellow's progress, using a report built from every reviewed scan. It is led by the assistant program director.
